Chester Clinic
July 20th, 8am-11am
No Appointment Necessary
Free physicals are for grade school or high school students participating in sports for the 2023-2024 school year. NO COLLEGE students will be seen. Physicals are provided by family healthcare providers from Memorial Hospital's Rural Health Clinic.
A SIGNED FORM IS REQUIRED: Parents are asked to fill out their portion and sign the physical form prior to the patient being seen (www.mhchester.com/free-sports-physicals)
